About Your Visit

Insurance Information

Dr. Mayoza is a provider for:

Dr. Mayoza is a certified Independent Medical Examiner for the State of Oklahoma, and accepts patients who have been injured in motor vehicle accidents.


Please call our office in advance to make an appointment. If you are unable to keep your appointment, we respectfully request you cancel your appointment 24 hours in advance. Physical therapy appointments are available by appointment Monday through Thursday. Patients with an emergency can be seen immediately during our regular clinic hours.

Fees & Payments

Unless previous arrangements have been made, payment is due at the time services are rendered. If you have health insurance, we will collect your co-payment/co-insurance, or deductible at the time of your office visit. If you have been injured in a motor vehicle accident by another party, no payment is required upfront. Please contact our business office for further information.

Prescriptions & Renewals

Please request any new prescriptions or refills during our regular office hours.  Renewals requested after office hours will be filled the next business day.

For Your Appointment

If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:

  • Driver's License or State ID
  • Referral (If Required by Insurer)
  • Insurance Information
  • Copies of Operation Records, Medical Records, X-rays, MRIs, and CT Scans from Prior Doctor Visits
  • A List of Current Medications and Any Known Allergies
